  • How can I confirm if my benefits are accepted?
    We accept a variety of health insurance plans. We can check benefits in advance of your appointment and bill on your behalf. Please Email me or call (844) 204-0524 with questions.

  • Can I be seen if I don’t have Insurance?
    Yes, we see patients that do not have insurance at a private pay rate.
  • What If I can’t afford a Private Pay Rate?
    What If I can’t afford a Private Pay Rate? Gateway to Rejuvenation LLC offers a sliding scale rate based off income. How do I apply for a sliding scale rate, how does it work? You will receive a sliding scale form with your intake packet. If for some reason you don't receive it, please contact us at admin@2rejuvination.com. Once received, you will be emailed an application. Please complete that application to the best of your ability and once completed, return it to your therapist or admin staff for review and approval. You will be notified if you qualify or not for a sliding scale rate.

  • How can I book a Therapy appointment?

  • What If I call to book an appointment and there’s a waiting List?
    Before you are placed on the waiting list you will be informed how long the wait is at that time, it will be up to you if you would like to be placed on the list or not. If you are placed on the list, as availability becomes available you will receive an email or a phone call before we move on to the next person on the list.
  • What is our Cancellation Policy?
    Cancellations must be made 24-48 hours prior to your scheduled appointment.
  • What If I miss my appointment?
    Missed appointments will be $35 per missed appointment. The amount due must be paid in full before you can schedule your next appointment.
  • How much time is each session?
    Therapy Session’s are from 30-50 minutes.
  • What are the days and times of operation for Gateway to rejuvenation?
    Monday-Friday 6am-5pm 2nd & 4th Saturday 8am-4pm Closed on Sunday
  • What to expect in sessions?
    Lola offers a 5-10min free consultation, during that time you may ask any questions, she will explain how it works. In addition, Lola will inquire about your needs and explain how she can best assist you.
