- 06What If I can’t afford a Private Pay Rate? Gateway to Rejuvenation LLC offers a sliding scale rate based off income. How do I apply for a sliding scale rate, how does it work? You will receive a sliding scale form with your intake packet. If for some reason you don't receive it, please contact us at admin@2rejuvination.com.(admin@2rejuvination.com) Once received, you will be emailed an application. Please complete that application to the best of your ability and once completed, return it to your therapist or admin staff for review and approval. You will be notified if you qualify or not for a sliding scale rate.
